In all of that work, the procedures exist because of incidents that happened years ago to people the current crew never met. That is the strange thing about high consequence, low frequency work. The better it is run, the less evidence anyone has that the rules matter.

So complacency here is not carelessness. It is the natural product of long success. A step gets skipped once with no consequence, which teaches that the step was optional, and after enough repetitions the shortcut becomes the method. Nobody decided to be unsafe. The environment taught them.

Which means leaders cannot rely on incidents to keep people sharp, because incidents are the thing being prevented, and they cannot run on fear indefinitely either, since fear fades faster than a career lasts. What actually sustains vigilance is making the reasoning visible. Not just what the procedure is, but the story of why it exists. Treating a near miss as valuable information rather than as a paperwork problem. And leaders who ask what would have to go wrong here rather than only whether anything did.

Health care lives the same pattern with hand hygiene, time-outs, and checklists, which is why this transfers cleanly between a refinery all-hands and a hospital leadership day. The five hundredth time is exactly when it stops feeling necessary.
