Healthcare dominates employment here at a scale that is hard to picture. The eight largest health systems and hospitals in the city employ just under three hundred thousand people between them, roughly a third of the combined workforce of the city’s twenty five largest employers. Northwell Health is New York State’s largest private employer with more than eighty five thousand people across twenty eight hospitals and over a thousand ambulatory sites. Now think about what that scale does to a person. In a hundred person company, the chief executive is the culture. Everybody sees them, reads their mood, hears them in a hallway. In an eighty five thousand person system, the chief executive is a name at the bottom of an email. A nurse working nights in Staten Island will never meet that person, and nothing they do on a Tuesday changes anything about her Tuesday.
What determines her Tuesday is one charge nurse. That person schedules her, decides whether her concern gets escalated or dismissed, notices or does not notice when she stays late. As far as her actual experience of employment goes, that supervisor is the company. The system-wide engagement initiative, the values statement, the town hall, all of it reaches her through that one filter, and if the filter is bad the initiative is noise.
So the practical conclusion for a large New York employer is uncomfortable. Money spent at the top of the organization to improve culture largely evaporates before it reaches the floor. Money spent making frontline supervisors better at the specific behaviors of leading people is the only version that arrives.
